FamPACT (Family Planning Access Care Treatment) www.familypact.org
Who is Eligible?
Women at risk for pregnancy and men at risk for causing unintended pregnancy; all California residents with incomes at or below 200% of the Federal poverty level with no other source of family planning health care during childbearing years
What is Available?
Contraception, fertility awareness, pregnancy testing, female and male sterilization and reproductive health education and counseling. Additional services include testing and medical treatment for STDs, cervical cancer screening, HIV testing, and as of Jan. 1, 2002, screening mammograms. Some blood work/lab tests covered when medications used for contraception (see program manual). Covers OCPs which we dispense from clinic, but can also be filled at certain pharmacies.

Who is Eligible?
All Medi-Cal patients in Santa Barbara except special categories below are covered by the Regional Health Authority (CenCal Health).
What is Covered?
All medical and surgical treatments and laboratory testing if the supplier is contracted with the Regional Health Authority.

Who is Eligible?
Medi-Cal patients who are not part of the capitated plan of the Regional Health Authority (CenCal Health)
What is Covered?
All medical and surgical treatments and laboratory testing as long as the supplier accepts Medi-Cal payment. State Medi-Cal will be billed as FQHC charges.

Who is Eligible?
Persons age 65 and above who contributed to the U.S. Social Security System and applied for benefits; also some permanently disabled persons will qualify at younger ages.
What is Covered?
Hospitalization covered with deductible and co-payment for all (Part A Medicare); Outpatient costs covered with deductible and co-payments if member pays a monthly premium or they may be eligible for Medi-Cal to cover this portion (called “Medi-Medi”). Private insurance companies have now contracted with Medicare to provide prescription drug benefits (Medicare Part D). Most Medicare Part D plans are accessible on the internet. Patients pay a monthly fee, which varies from carrier to carrier. If the patient is “Medi-Medi” there is no monthly fee.

Any Medicare beneficiary who signs up with HMO health plans (called Medicare Choice plans)
What is Covered?
Everything Medicare traditional Fee for Service covers plus more that the health plan designates, such as prescription drug coverage.

Who is Eligible?
Employed persons and their families who have this insurance purchased through their employment (non Medicare)
What is Covered?
Most medical and surgical services including preventive care as defined by the contract their employer purchases; usually includes prescription coverage with co-payments.
CHDP (Child Health and Disability Prevention Program) and Gateway www.sbcphd.org/cms
Who is Eligible?
Children birth through age 20 who are in Medi-Cal OR meet income eligibility category A & B, or in Head Start or State Preschool Program. Legal Residents are enrolled via the internet through the CHDP Gateway Program that classifies the child with presumptive eligibility immediately so they can receive follow-up CHDP services for free through Medi-Cal. Non-legal residents are still eligible for CHDP, but not Gateway, therefore any follow-up work will be paid for through EAPC instead of Medi-Cal/Gateway.
What is Covered?
Routine well childcare visits using CHDP periodicity schedule; extra well childcare visits can be justified if needed and partial visits to complete immunizations and lab tests are allowed. Any referrals noted on CHDP form will be followed-up by CHDP staff.
